Regarding Financial Policy - Credit Card on File for All Balances

Overview
At Norwood Family Eye Care, we are committed to providing you with the best possible eye care services and ensuring a convenient and hassle-free experience. To streamline our payment process and enhance your convenience, we have implemented a new payment policy using a Credit Card on File system for any balances remaining after insurance has been paid or any balances due to the practice. Your credit card information will be encrypted and stored securely at our trusted payment processing partner, Fullstream.

Hassle-Free Billing
With the Credit Card on File system, you no longer need to worry about manual payment
processing, paperwork, or remembering payment deadlines. It streamlines the billing process,
making it more convenient for you.

Credit Card Authorization
By receiving care at Norwood Family Eye Care, you agree to provide and maintain a valid credit card on file. This credit card will be used to cover any outstanding balances not covered by insurance or that may be due to Norwood Family Eye Care.

Insurance Billing
We will file insurance claims on your behalf for the services provided. Once your insurance company has processed the claim and provided payment, we will determine the amount of any remaining patient responsibility, including deductibles, co-pays, or non-covered services.

Credit Card Usage
When you check out after your appointment, your credit card on file will be automatically charged for the services rendered. This ensures that you pay only what you owe based on your insurance benefits and the services received. If there is an outstanding balance after insurance has been paid, we will charge the remaining balance to the credit card on file. The card will be charged only after you have been notified of the amount and received an explanation of the charges.

Notification
You will receive an explanation of the charges and notification of the credit card charge via email, text message or postal mail. It is your responsibility to ensure that the contact information we have on file is accurate and up to date.

Dispute Resolution
If you believe there is an error or discrepancy in the charges billed to your credit card, please contact our office immediately to address the issue. We will work with you to investigate and resolve any disputes.

Refund Policy
If there is an overcharge or billing error, we will promptly issue a refund to the credit card on file. Refunds will be processed in accordance with our billing policies.

RETURNED CHECK FEE
While it is not our policy to routinely accept checks at NFEC, from time to time a payment in the form of a check will be accepted on a case-by-case basis. If the check is deposited into our account, and it is returned for insufficient funds, you shall be charged the full amount of the check written, plus bank service charge and a non-sufficient fund (NSF) fee of $30. If there is a credit card on file your card will be charged for those amounts. If your balance must be sent to a collection agency, you will be assessed fees associated with collections.

We prioritize the security of your data, and rest assured that your information will be kept confidential. We believe that this policy will enhance your overall experience with Norwood Family Eye Care by reducing administrative hassles and ensuring accurate and timely payments.

Updating Credit Card Information

You are responsible for notifying us promptly if there are changes to your credit card information, including card expiration dates or changes in billing address.

This policy complies with all applicable laws and regulations, including the Health Insurance Portability and Accountability Act (HIPAA) and the Payment Card Industry Data Security Standard (PCI DSS).

By receiving care at Norwood Famil Eye Care, you acknowledge that you have read and understand this financial policy and agree to comply with its terms.
